The default GPG command options for fine for me here, on both sending
and receiving signed+encrypted email.

ii  mutt           1.5.4-1        Text-based mailreader supporting MIME, GPG, 
ii  gnupg          1.2.1-2        GNU privacy guard - a free PGP replacement.

My pgp_encrypt_sign_command is set thusly in /etc/Muttrc:

set pgp_encrypt_sign_command="/usr/lib/mutt/pgpewrap /usr/bin/gpg --charset utf-8 --passphrase-fd 0  --batch --quiet  --no-verbose --textmode --output - --encrypt --sign %?a?-u %a? --armor --always-trust -- -r %r -- %f"
