PCI ID 8d04 is the ProSavage-DDR. This is a relatively new motherboard-based
entry into the Savage family. It did not exist when Xfree86 4.2.0 was
released, so the stock driver in XFree86 4.2.0, which is what you are running,
does not support it.
Go fetch the 1.1.23t driver from my web site,

By the way, you might as well delete the module load lines for GLcore, dri,
glx, pex5, and xie. The latter two are obsolete, the first three have no
Savage support.
