You need to apply the patch before building the deb, obviously.
Don't use the '-b' option to apt-get.

  cd /foodir
  apt-get source xxxx
  cd xxxx-*

  >> apply patch <<
  
  fakeroot debian/rules binary
  su -
  dpkg -i /foodir/xxxx*.deb
