You would have to rewrite the header in a router rather than using the
general header rewrite section of the config file. In other words, when
exim detects that the address is non-local, it would look for a router.
The first router should probably re-write the header. It is possible, and
that is the method that you would need, just exactly sure how to do it but
I can look it up in the documentation for you later if you can't find it.
You might ask on the Exim mailing list.  

For local mail, exim will use a director rather than a router and the
address would be left unchanged.
