upgrade the freediams source package ? or the binary package ?
If you are questionning about source package, can we create something like a virtual/fake source package fully depending on the freemedforms-project one ?
For the binary, there are no problem.

External EMR should keep the freediams dependency, there should be no need to change anything about the binary package if the freemedforms-project source package is uploaded.

I have to tell that I'm not a Debian guru and that I've already spent many, many hours on this work. The freemedforms-project source package is fully functional without any patch. I'll not be really enjoy to spend another hours on the packaging while it already operates on Launchpad. But I can perfectly understand the need to avoid "brutal break" in source package, and will be pleased to help on this question.

What are your recommendations ?
