I'm not so good with this stuff but it seems to me you are loading two modules 
here: 3c575_cb which fails and a 3c59x which does work. Now I don't know 
which one is the correct one, but you definitely need to check your 
/etc/network/interfaces to see if your card actually get started.

Since you have two modules loaded, it might (MIGHT) be that you have both an 
eth0 and eth1, where eth0 is the failing module and eth1 is actually what you 
want...

Easy to check out: update your /etc/network/interfaces and do an ifup eth1, 
see what happens to those greenlights on your hub.
