We just immediately roll out new packages for urgent problems, such as
security releases, upgrading on test/dev servers first to make sure that
nothing is obviously wrong.  For other security updates and stable
packages, we apply all pending upgrades for every system at least once
every three months.  We track how long it's been and make sure we touch
each system at least that often.

I've never had any trouble with just applying Debian stable updates
(unlike Red Hat updates, which have from time to time broken things).
