In many cases, bugs are already filed on ftp.debian.org.

These look like NMUs:

IMHO, if someone is doing an NMU which is not just a recompile, i.e.,
not versioned as a recompile, then it must be done with source, or
else dinstall should reject it.

Moreover, all the things you mention here except for the 'recompile'
NMUs should be considered RC bugs.
