Please read Section 6.2 from the Developer's Reference.  There are
variations for various situations:

     6.        Moving, Removing, Renaming, and Orphaning Packages
     6.1.      Moving packages
     6.2.      Removing packages
     6.3.      Replacing or renaming packages
     6.4.      Orphaning a package
