Bug#281395: text frontend title handling improvements

The text frontend could make some simple changes to improve how it
displays titles. Curently it displays the title before every question:

Title
Question
Prompt: '?' for help> foo

Title
Question
Prompt: '?' for help> bar

This looks pretty strange IMHO since the title often looks like it's
part of the question. Also why display it over and over each time?

debconf's readline frontend only displays the title before a question if
it is different from the title it displayed last time. And it sets the
title off from the question:

Prompt: '?' for help> foo

Question
Prompt: '?' for help> bar
