For reference 3/0C/08 stands for "WRITE ERROR - RECOVERY FAILED." According to specification such error codes are returned when "extended error reporting" is on (it's not on according to dvd+rw-mediainfo verbose output!) and kind of defined in rewritable context only (you're referring to write-once recording). The only recovery procedure which takes place in DVD-R context is recovery from buffer underrun. But as you record at 1x probablity is minimal. But one way or another if it fails to recover from buffer underrun, I'd say it's media problem in this case. As pointed out on the list, bad batch is a possibility. A.
