As far as I know most use cables cut to length and terminated at the
factory for in-building runs (though it is certainly possible to do it
in the field).

I believe that most connector systems use a spring to push the fiber
ends together. Otherwise temperature changes could create a gap.
In any case while in-building fiber may be becoming common in large
organizations it is still rare in small ones.
