    CVT is enough different from GTF that it should not be used on monitors
    that aren't expecting it.  This brings us closer to what the spec says
    the correct behaviour is.

So using CVT when we want GTF doesn't seem right (I haven't actually
looked at what the vesa driver used to do, so I need to check that).
