SOLVED ...
Short story:
regenerate exim.crt and exim.key
Turns out that most servers out in the world were happy with the re-use
of the old exim.crt and exim.key files.
These servers had problems until I created new key and cert files:
Microsoft Hotmail [live.com, live.com.au, hotmail.com, outlook.com]
and also
Any services going through Websense [mailcontrol.com servers]
I had an old 10 year 2048 bit RSA cert, it is now a 5 year 4096 RSA
cert. These certs are self-signed too, being self-signed wasn't the
problem though.
