The same way :-)
# apt-get install smart-notifier
That will also install smartmontools.

# cat /usr/share/doc/smartmontools/README.Debian
Longer version:-
#cat /usr/share/doc/smartmontools/README
When you've configured smartmontools, you set up smart-notifier
# man smart-notifier
