I didn't think the favicon is significant.  firefox is just looking
for an icon.  it's the Premature end of script headers: qmailadmin.

I've done several reinstalls and get the same problem.
this is my configuration script:
./configure \
--enable-ezmlmdir=/home/user/ezmlm-0.53 \
--enable-cgibindir=/var/www/cgi-bin \
--enable-htmldir=/var/www/html/ \
--enable-imagedir=/var/www/html/images/qmailadmin

should the cgi-bin be under html.  I don't think so... but it would
make sense since "mysite.com" is being pulled from
"/var/www/html/mysite"

I found this link http://www.opensubscriber.com/message/qmailadmin@inter7.com/2050391.html however it is related to version 1.2.8 and is supposed to have been fixed in 1.2.9. I did download 1.2.9 and installed it on one of my machines and it is working. I have my cgi dir as /usr/lib/cgi-bin and html as /var/www I don't think it will matter though provided that you have apache pointing to them correctly. One thing that might be worth trying is an older version of qmailadmin to see if it makes a difference, qmr has 1.2.3 so that would be a quick check, if that works maybe redownload 1.2.9 from sourceforge and try again.
And yeah the favicon is no big deal, just annoying when fills up your logs.
