zcat /usr/share/doc/mutt-1.5.6-r5/smime-notes.txt.gz | less.

You need to use the smime_keys perl script and you will need openssl
installed (since smime_keys is just a wrapper script).  You will then
need to set various variables in your .muttrc. This is all explained in
the smime-notes.txt.
