Now, the confusion lies in the last paragraph of the quote. Is this the way to set an environmental variable?

No, this just sets a variable in the file /etc/default/mailscanner, which in turn is read (and needed) by the script /etc/init.d/mailscanner.
Do I use the set
command?

No.

The problem is that the mailscanner package is not even _installed_ properly, otherwise the file /etc/default/mailscanner would already be there and await your editing.

I _strongly_ suggest you de-install mailscanner, `apt-get --purge \
remove mailscanner`, do an `apt-get update; apt-get -f install`, and _only_ if that finishes without any errors whatsoever to re-install mailscanner with `apt-get install mailscanner`. Then, let us know about your progess ;)

Robert, here are the result of the above suggestions:
apt-get --purge \ remove mailscanner said mailscanner
was already uninstalled.  I had successfully removed it
before.  good so far.
Did the apt-get update OK here.
Did apt-get -f install, no errors unless "7 packages not upgraded
would be considered and errror.  I don't usually consider it so.
Then I did the apt-get install mailscanner with the listed output
below:
unpacking mailscanner...
/var/lib/dpkg/tmp.ci/preinst line22: db_get: command not found
(note: this file doesn't exist. I looked)
dpkg: error processing /var/cache/apt/archives/mailscanner_4.37.7-1_all.deb(--unpack)
Sub process pre-installation script returned error exit status 127
Errors were encountered installing mailscanner
E. Subprocess /usr/bin/dpkg returned error code (1)

I believe the above is an exact quote  of the  mailscanner install  output.
Hope it help to pin down the problem cus I'm at a loss to fix it.
