Hello,
if you use kernel-package, you can just run this:
make-kpkg --added-patches foo ...
See man make-kpkg for more details.
If you want to patch your kernel source tree permamently (or there is no 
debian package for the patch), you can use this commands:
cd /path/to/kernel/source
cat /path/to/patch|patch -p1

If you use ext2:
mke2fs /dev/hdXn

If you prefer ext3:
mke2fs -j /dev/hdXn
