Yes. One file per student, one line per question, of the following
format:

  01: ab
  02: ce
  03: d
  04: acd
  [...]

Yes. There are web-based systems (which I may use next time), or
complete suites, but nothing that can just do the analysis without
administering the actual exam... and I think I looked quite closely
(which is what I usually try to do -- the homework -- before posting
to d-u...)
