Hi Mike,

   I'm unaware of a .deb package that includes the files for a web
survey, but I used this and thought it pretty good:

     <http://phpesp.sourceforge.net/>

It's in PHP, of course, so you should just be able to install the PHP
+ Apache .deb packages and then configure phpesp by hand.
