Sounds like your local mailserver is slightly misconfigured. Reason Balsa works is that it implements sending mail whereas Mutt uses the local MTA for sending. I have had the same problem as you and fixing my Exim configuration solved the problem. I'll send you my exim.conf (in private e-mail to not bother the entire list with it) so can compare it with your own using diff. If memory serves me well, the problem was in the definition of my transports.
