I think I've solved the problems with courier now.  Apparently the mail
directive in the userdb is completely useless.  I symlinked Maildir in
my $HOME to /var/spool/maildirs/hmike and it seems to work.  I'm going
to file a bug report unless anyone says otherwise. courierauthtest got
me thinking about this because it never reported the mail directory
properly.
