I've got this mouse working using:

Section "InputDevice"
	Identifier	"Generic Mouse"
	Driver		"mouse"
	Option		"CorePointer"
	Option		"Device"		"/dev/psaux"
	Option		"Protocol"		"ImPS/2"
	Option		"ZAxisMapping"		"4 5"
EndSection

This is with XFREE86 4.0.x

The wheel generally works.  The thumb button duplicates button 2.
