These are kernel, not fdisk, errors.  fdisk may be triggering them, but
they're coming out of the kernel.

Can't help you beyond that.

It would be helpful to know what kernel you're using.

This would appear to be a SCSI driver problem, the message originates in
/usr/src/linux/drivers/scsi/sd.c (2.2.15).
