Hi Ken,
there is some floppy controller hardware out there
which fails to inform the kernel when the floppy
was changed.
On the debian distro is a utility called "fdflush"
(in its own .deb i believe) which fakes the "fl has
changed" message to the kernel.
So 1. change floppies
2. call fdflush
I had similar problems, this works fine for me.
