As root, do 'cd /dev; MAKEDEV ttyS1'  Note the upper case 'S'.

Don't do that.  Use /dev/ttyS1 directly.

That documentation is out of date.  Creating a /dev/modem symlink can cause
locking errors, and the cuax devices are obsolete.

May I suggest that you try pppconfig?
