cgi script for dos2unix and unix2dos?

Anyone have a cgi script for the dos2unix and unix2dos commands?

I'm not talented in html or cgi scripts, so I can't exactly make one.
Here's how I thought it might work:

1) web page:
     2 radio switches, 1st=dos2unix, 2nd=unix2dos
     2 small text fields
     "ok" button at the bottom of page
   the 1st small field is for reading a file
   the 2nd small field is the location to write a file
   "ok" goes to the next step
2) asks for user name and password
3) runs dos2unix or unix2dos
4) saves file

can anyone help me out?
