Turn on debugging on chat and pppd and see how far it gets. (chat -v
and `pppd debug kdebug 1' for almost everything.)

These are Appletalk and IPX. It's quite unlikely that you want these.
In fact, you should probably alias them off in /etc/conf.modules.

CHAP is part of pppd. AFAIK it should get used automatically if
chap-secrets is set up properly.
Does chat actually connect properly? You should see something like
the following, even if CHAP doesn't work:
Aug 7 18:15:35 psyche pppd[308]: pppd 2.2.0 started by carey, uid 1000
Aug 7 18:15:54 psyche pppd[308]: Serial connection established.
Aug 7 18:15:55 psyche pppd[308]: Using interface ppp0
Aug 7 18:15:55 psyche pppd[308]: Connect: ppp0 <--> /dev/ttyS1
Sometimes my ISP hangs up here, when PAP doesn't work for some reason.
If the line is busy or similar, I get:
Aug 6 19:01:37 psyche pppd[2002]: pppd 2.2.0 started by carey, uid 1000
Aug 6 19:01:47 psyche pppd[2002]: Connect script failed
Aug 6 19:01:47 psyche pppd[2002]: Exit.
