Aemmmm, thanks for your document ... but it is well, complicated.

Can you provide a *minimal* exmaple? Looking at your code I see
about several places that looks rather fishy, makeatletter, changing
internal stuff, there is no guarantee that this works.

Then, you are using about 35 different packages??? 

It might be that some of these packages are not compatible.

Please, send a *MINIMAL* document, and that means also minimal with 
repect to loaded packages, that exhibits this behaviour.

Not all packages in the TeX world are compatible with each other,
and your mixture seems to be really crazy.

BTW, I tried to find out where it might come from, but I really
believe it comes from your horriblie mixture of packages.
I couldn't nail it down, because it also depends on the engine
one is using, so that is a very unclear problem.
