It is hard to know without knowing more about DICOM, but it sounds
like the DICOM server is similar to SANE (the scanner daemon). With
SANE the user GUIs ask the user where to store the gathered images.

As an example of another daemon that stores data in /var, mysql
prompts the user via debconf in postrm to decide to remove the data or
not, defaulting to not removing the data. As long as the DICOM server
is storing stuff in /var that sounds like a reasonable default.
