Actually there is also:

  1a. The developer wrote Makefiles by hand, but also happens to have a
      clue, so at some point added DESTDIR support.

You can spot this case, generally, with 'grep DESTDIR Makefile'.  Of
course there is the possibility that the developer only had half a
clue, so this is broken, but then it's just a bug to be patched and
handed upstream.
