You need to run lh_config, lh_bootstrap, lh_chroot and lh_binary. After
lh_chroot all apps in config/lh_chroot (some section there) are
installed and you can configure them within the chroot dir. The last
command builds the ISO.
I also recommend to read the debian live wiki. It is in some parts
outdated, but others are still useful.

You don't need a password for root. Simply type "sudo bash" as user.
