That's the header from a different file. :)

As you showed above, 06_replace_mencoder_with_ffmpeg_in_manual.dpatch is
fine, which is why Lintian isn't warning about it. Lintian is warning
about 04_python_2.6_compatible_setup.py which starts as follows:
#! /bin/sh /usr/share/dpatch/dpatch-run
## 04_python_2.6_compatible_setup.py.dpatch by <pietro@sidousci.vousci>
##
## All lines beginning with `## DP:' are a description of the patch.
## DP: No description.
