I've seen similar things on my Dell I4000 - but only after using hotswap
- e.g. removing the cd-rom drive.  Same symptoms. I learned the hard way
to make sure that I rmmod'ed ide-cd *before* removing the cdrom drive.

I only realised why after browing /var/log/syslog (or was it by looking
at the console - it's a while back...)

The only way to get things back to normal was to re-insert the cdrom
drive, wait until the kernel was happy, and *then* umount + rmmod +
remove cdrom drive.

Luckily, hotswap (from the package of the same name) seems to do things
right.
