Either surplus that ("choose which one(s) ddclient should...") or a
misused "which" ("choose the one(s) that ddclient should...").

Or to shut me up with my moaning about how (strictly speaking)
ddclient doesn't update hostnames, you could say it this way round:

 _Description: Host names to manage:
  The list of host names managed via your DynDNS account has been
  downloaded.  Please choose the one(s) for which ddclient should be
  used to keep IP address records up to date.
