Did you verify that both checkboxes and text fields were preserved?  I
have vague memories of the text field value put into
<URL:http://www.nav.no/binary?id=268288&download=true> moved when the
file was first saved and next loaded from disk.  The printed result
had problems too.  Do not remember the details any more, as we had
other things on our minds at the time and did not report this to the
developers. :(

I must admit that this was my first and only time using PDF forms. :)
