I actually copied it from the old RedHat 7.2 for Alpha CD I downloaded from
Digital/Compaq/HP via this link:
http://h18000.www1.hp.com/products/software/alpha-tools/redhat/download.html
You could also use the one from AlphaCore 1.0. which would also work. I
know it works because I have one of my ES45's up running the latest
AlphaCore from www.alphalinux.com.
Are you using hba's on yours?
