Unfortunately, patches to two of the files didn't apply.  It looks like you
weren't diffing against original sources; other Debian patches were not the
cause:

I'm applying the other two to 4.0.2-11 as patch #600.  You can find the
broken half in held-patches/600a.diff when I release.
