I used the:
alpha-patches-2.0.35-0.2.gz
from gatekeeper
Upgraded with the 0.90 tulip driver which solved some bad autonegotiation
problem (in particular, the previous versions were not working as is
on Miata).

>From the kernel-patch package:
 misc.diff.gz (to compile with -O)
 deb-reverse-2.0.35.diff.gz (implicit)
 
I think that the alpha-patches-2.0.35-0.2.gz take care of the
alpha-dma.diff.gz patch and alpha-patches-wo-tulip-2.0.35.diff.gz

I have not applied 2.0.36-pre-patch-12.gz, not sure it it applies
cleanly with alpha-patches-2.0.35-0.2.gz (not tried)

As for the kernel configuration, I started from the intel
configuration for generic things (but compiling in the kernel the
network module and nfs, instead of modules), for drivers I tried
to do a merge between the old kernel configs available in the set
from October 1997, and the configuration used in the kernels on
gatekeeper. The precise way I generate the .config is on my web page.

I cannot promise anything soon, I guess we need a specific Alpha
step in the install program to create a fat partition for MILO.
Then initializing MILO should be straightforward.
