DVD+RW kernel patch - scsi error (help!)

I am having a problem with burning DVD+RW's with my new DVD recorder.
After patching the 2.4.18 kernel with Andy Polyakov's DVD+RW kernel patch,
I am getting the follow error when I attempt to 'mkudffs --spartable=2
--media-type=cdrw'

Sep 1 17:33:32 www kernel: scsi0: ERROR on channel 0, id 0, lun 0, CDB:
Request Sense 00 00 00 40 00
Sep 1 17:33:32 www kernel: Current sd0b:00: sense key Medium Error
Sep 1 17:33:32 www kernel: Additional sense indicates No seek complete
Sep 1 17:33:32 www kernel: I/O error: dev 0b:00, sector 9180160
Sep 1 17:33:40 www kernel: scsi0: ERROR on channel 0, id 0, lun 0, CDB:
Request Sense 00 00 00 40 00
Sep 1 17:33:40 www kernel: Current sd0b:00: sense key Medium Error
Sep 1 17:33:40 www kernel: Additional sense indicates No seek complete
Sep 1 17:33:40 www kernel: I/O error: dev 0b:00, sector 9180032
Sep 1 17:34:24 www kernel: sr0: injecting "SYNCHRONIZE CACHE"
Sep 1 17:34:32 www kernel: sr0: dirty DVD+RW media, "finalizing"

Running a 'mount -o rw,noatime /dev/scd0 /mnt/cdrom' and trying to copy
information to the volume invariably leads to failure. I am running the
kernel 2.4.18, but have seen this error under 2.4.21 and 2.4.22, as well.
Does anyone know what could be happening, here? I can't make heads or
tails out of this message.

Strange thing: I successfully made three backups without error when I
first installed this drive.

Thanks in advance!

-Frank
