You'll probably be able to build libnbd for macOS:

https://gitlab.com/nbdkit/libnbd

It would give you various userspace clients, which may be useful
depending on what you're trying to do.

As Wouter has said, porting the kernel client would be very difficult,
essentially a from-scratch rewrite.
