For every package APT will upgrade that is not specified on the
commandline, it will say "extra packages will be installed" (in
fact, for any package that will be installed/upgraded that is
not mentioned on the command-line).

This is ~slightly~ incorrect, but I don't see why you are confused,
as it can obviously do only one of those things.

sorry for top post.

Why I'm confused? Probably because it removes other unrelated packages which were not removed using manual steps.
