For every package APT will upgrade that is not specified on the
commandline, it will say "extra packages will be installed" (in
fact, for any package that will be installed/upgraded that is
not mentioned on the command-line).

This is ~slightly~ incorrect, but I don't see why you are confused,
as it can obviously do only one of those things.
