The error is fixed, you can download the file again from
http://www.debian.org/doc/user-manuals#quick-reference.

Items 1, 2 and 3 are still open. If really nobody cares about it I will
propably do it next weekend (it's not urgent).

Yes that's fixed now. But I wonder whether menu.inc is really used ...
