I've never seen this before, but I searched the rsync(1) man page for
'+', and I'm fairly sure this is output of the type requested by the
'--itemize-changes' option (abbreviated as '-i').

Did your rsync invocation include either of those arguments?

If yes, I recommend you read the man page under the explanation of that
argument.

From the explanation I see there, the character sequence you gave as
"some of" would mean:

< - an item being transferred to the remote host
. - the item is not being updated
+ - the item is newly created
f - the item is a file
c - the checksum of the item is different from what it was before
t - the modification timestamp of the item is different
d - the item is a directory
s - the item is a symlink

and since some of those are mutually incompatible, I can only presume
that these are taken from different entries.
