Please take a look (if possible), which font the PDF is using in the fields. 
You must have installed these fonts on your computer.

I ran into the same problem, when creating a PDF with Libreoffice with form 
fields, but the other person only had Windows and had not installed the fonts 
(Liberation Serif), so He could not read it.

Maybe same problem at yours? Just an idea....
