Purge unconfigured packages providing init scripts. That is, init scripts are considers conffiles so removing the package doesn't remove the file. Because the package is removed, it doesn't get updates to fix problems such as above. Step 2. If you have any init scripts that you developed yourself, add proper headers[1] to the script. Step 3. If you have any init scripts that are provided by a third party, add the proper headers to a file in /etc/insserv/overrides. The filename must match that of the init script (not the symlinks thereto).
