Have a look at XMPP. The Jingle extension allows Voice and Video conversations and the Proxy65 extension allows file transfers. As with OCS, you can run your own server for access only by office staff. Prosody is a good server to start with, but ejabberd is somewhat more powerful and scalable. In terms of client, any XMPP client will do (Pidgin, Empathy etc).
