I don't use pulseaudio but I would try:

1/ Start a session with a new user and check if it can hear anything

2/ Review the logs, usually "~/.xession-errors" and "/var/log/syslog"

3/ Open an audio player from command line and play a song, then check for 
any output messages

4/ Recheck the audio control levels and setup
